All agencies are required to comply with their FY2017 contract, Section II General Requirements- Reporting. Each agency must submit the Program Evaluation form to the Board semiannually. Please refer to the Program Evaluation form for reporting dates.

All agencies funded through local levy funds are required to submit with the invoice their financial match for services. The financial match needs to be listed on each invoice.
